The role requires caseload management, completing Care Act Assessments, reviews and proportionate support planning, including Mental Capacity Assessments and Best Interests decisions.
You will work with health, provider services and safeguarding colleagues, with hybrid working and two days in the office per week. A car and Social Work England registration are essential.
