Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on 1 April 2026. If your start date is on or after 1 April 2026, you will be employed by the newly formed organisation. If you join Bridgewater prior to this date, your employment will automatically transfer into the new organisation. Job Purpose

The post holder is responsible for the overall management of the Treatment Room services across Halton and for standardising leg ulcer care across Halton. The post holder actively supports the Treatment Room Service by undertaking responsibility for delegated patients within a clinical setting. The post holder is responsible for the Treatment Room Nurse leadership and for ensuring the training needs of the Treatment Room Staff are met, including PDPs. The post holder is responsible for data collection for undertaking audits.

Responsibilities

  • Undertakes the assessment, planning and evaluation of patients care needs.
  • Liaises with the District Nurse Coordinator and District Nurses to ensure information sharing in relation to patient care, changes to care plan and continuity of care, and review of complex care packages.
  • Mentoring, Teaching and Assessing.
  • Liaises with all health care professionals and other agencies to ensure effective service delivery.

Duties and Responsibilities

Clinical:

  • To promote the health and wellbeing within the family unit and community as a whole.
  • Perform comprehensive assessment of patient nursing needs as the named nurse.
  • Plan, assess and evaluate programmes of care according to changing health needs under the direction of the wound care specialist Nurse.
  • Maintain accurate records in accordance with NMC.
  • Communicate and collaborate with other PHCT members, voluntary and statutory organisations to support individual patient care.
  • Establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
  • Competently perform clinical skills related to the post.
  • Be professionally responsible for a delegated caseload.
  • Assess health needs at individual, practice and community level.
  • Work without direct supervision.

Managerial

  • Access support from the District Nurse Coordinator.
  • Manage the Treatment Room Rota/phlebotomy Rota across Warrington.
  • Manage the training needs and competencies of the Treatment Room Staff/phlebotomy staff.
  • Manage sickness/absence of the Treatment Room staff/phlebotomy staff.

Clinical Governance

  • Support clinical supervision within the team.
  • Maintain professional portfolio that demonstrates evidence of ongoing professional development.
  • Ensure practice is evidence based through undertaking audits and critically appraising literature.
  • Maintain clinical standards through reflective practice and clinical supervision.
  • Adhere to the NMC Standards: Professional Code of Conduct; Scope of Professional Practice; Administration of Medicines; Records and Record keeping.
  • Identify any limitations/gaps in professional or clinical knowledge.
  • Contribute to the development of and implementation of policies, procedures and guidelines relevant to own area of work.

Teaching and Learning

  • Act as a practice assessor for pre-registration nursing students.
  • Act as a mentor for new treatment room staff.
  • Support the development of the nursing team by teaching and assessing junior members of the team.
  • Undertake mandatory in-house training and other training that supports the role.
  • Access clinical supervision.
  • Undertake training that is specific to the post.
  • Teach patients, carers and other professionals to support patients to maintain their optimum level of independence.

Personal Issue

  • Participate in the PDP process.
  • Adhere to all Trust Policies.
  • Report sickness and other absences to the identified administrator.
  • Provide accurate, timely information to the identified administrator regarding annual leave, expenses and study leave.
  • Supply required activity data related to patient care.
  • Adhere to the lone worker policy.
