The predicted salary is between 36000 - 60000 £ per year.

Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on the 1st April 2026. As such, if your start date is on or after 1st April 2026, you will be employed by the newly formed organisation. Should you join Bridgewater prior to this date, your employment will automatically transfer into the new organisation. Job Purpose

The post holder will manage a caseload of registered patients within a team or team(s). The post holder will delegate the workload to the community nurses, healthcare assistants and nursing assistants as appropriate and as defined by the priority/acuity of the caseload and liaise with all health care professionals and other agencies within the wider community to ensure effective service delivery. The post holder will work under the direction of the District Nurse Coordinator and will be responsible for the assessment of patient need. The post holder will develop, deliver and evaluate programmes of care. The post holder will support the care management of complex cases and further support leadership and development within the team. The post holder will play a part in mentoring pre and post registration students.

Responsibilities

  • Perform comprehensive assessment of patient nursing needs.
  • Plan, assess, implement and evaluate care delivery according to changing health needs utilising prescribing skills.
  • Support the care management of complex/continuing care packages.
  • Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.
  • Establish and maintain effective communication with individual groups and communities in complex and potentially challenging situations.
  • Work collaboratively with other professionals and agencies to ensure patients' needs are met.
  • Involve patients and carers in the planning and delivery of care.
  • Competently perform a range of clinical skills related to the post.
  • Maintain accurate records in accordance with NMC Standards.
  • Identify skills and knowledge to address individuals' training needs by undertaking personal development reviews and personal development plans within the KSF framework.
  • Participate in the health needs assessment of the practice population to identify areas of unmet need.
  • Ability to work without direct supervision and to solve problems.
  • Undertake chronic disease management reviews of the housebound patient using appropriate template.
  • Work on a rota basis to facilitate weekend working across the district nursing teams.
  • Managerial/Leadership
    • Work under the direction of the District Nurse Coordinator in providing nursing services to patients registered within a defined GP Practice.
    • Manage the night service caseload of patients.
    • Delegate the workload to appropriate team members.
    • Day to day responsibility for the designated caseload.
    • Provide accurate timely information to the identified administrator regarding annual leave, expenses and study leave.
    • Undertake back to work interviews and assist in monitoring and reporting to District Nurse Coordinator general HR issues surrounding staff sickness and absentees.
    • Report any concerns with regard to poor practice or concerns of staff members to the District Nurse Coordinator.
    • Support the effective use of physical and financial resources.
    • Order supplies and equipment.
    • Ensure as far as possible that the working environment is safe, adhering to the Lone Worker policy.
    • Provide leadership and support by promoting professional development and encouraging clinical supervision.
    • Supply required activity data related to patient care.
  • Clinical Governance
    • Support the clinical governance agenda by benchmarking best practice, identifying any inequalities in service delivery.
    • Ensure that practice is evidence based through undertaking audit and critically appraising literature.
    • Contribute to the development and implementation of policies, procedures and guidelines relevant to own area of work to ensure best practice.
    • In-depth knowledge of the legal, ethical, moral and accountability issues related to District Nursing.
    • Adhere to the NMC Standards: Professional Code of Conduct, Scope of Professional Practice and Administrations of Medicines records and Record keeping.
    • Support the organisations public health agenda.
    • Maintain a professional portfolio that demonstrates evidence of ongoing professional development.
    • Identify any limitations/gaps in professional or clinical knowledge.
    • Access and promote clinical supervision within the team; promote people's equality, diversity and rights.
  • Teaching and Learning
    • Ensure own continued professional development.
    • Undertake mandatory training and other training that is specific for the post.
    • Facilitate the placement of pre and post registration nursing students.
    • Support the development of the nursing team by teaching and assessing other members of the team including nursing auxiliaries.
    • Teach patients/carers and other professionals to support patients to maintain their optimum level of independence.
    • Promote lifelong learning with the nursing team.
    • Act as practice teacher to specialist practitioner students (NMC Standard 2006).
  • Records Management, Confidentiality and Security of Information
    • Adhere to Trust policies and procedures as directed in training, guidelines and advised by relevant colleagues in relation to creating records and handling information.
    • Undertake action as required to implement and comply with these policies and procedures.
    • Report any non-compliance.
    • Maintain confidentiality in relation to personal data held for colleagues and patients, ensuring it is processed lawfully; for no purpose other than for which it was obtained; is relevant to that purpose; is retained for no longer than is necessary; is processed in accordance with the rights of the subject to access and accuracy; and is protected from accidental loss or damage in accordance with the requirements of the Data Protection Act 1998, and records management guidance.
    • Maintain confidentiality of patient-identifiable personal data using a non-identifiable alternative, where practicable, and limiting access on a strictly need to know basis in accordance with the responsibilities of the Trust's Caldicott Guardian Codes of Conduct and Accountability.
    • Comply with relevant NHS codes of conduct and accountability, and professional codes of conduct appropriate to this role.
    • Take responsibility for own development including ensuring attendance at all statutory and mandatory training, maintaining personal development records and taking an active role within personal development processes.
