Post holder will work closely with the IV Team Leader(s) to manage a dedicated caseload of patients requiring IV antibiotics and long-term vascular access care. Care is delivered to patients in their own homes or clinic settings in accordance with protocols and guidelines. You will be expected to mentor community staff nurses during training for administration of IV Therapy.

Main duties

  • To perform peripheral cannulation and venepuncture in accordance with established protocols.
  • To perform port-a-cath procedures according to established protocols.
  • To act as an assessor to district nurses in the assessment of the administration of intravenous antibiotics.
  • To provide clinical support and advice to the patient and district nurses in the absence of the IV Team Leader (e.g. during holidays, study leave).
  • To work alongside the district nurse teams to support them in providing intravenous therapy within the patients' home environment.
  • To work under the direction of the IV Team Leader and within Trust policies and guidelines, following intravenous therapy patient care pathways.
  • To provide a safe, seamless transition of care by using a multi-disciplinary approach to care.
  • To work without direct supervision of the IV Team Leader in the patient's own environment and also within a clinic setting.
  • To support the assessment, development, implementation and evaluation of care.
  • To provide safe discharge of patient care from the pathway.
  • To provide accurate written communication of the patient's progress to the IV Team Leader.
  • To support the training/skills of competencies of other community nurses within intravenous antibiotic therapy.
  • Effectively communicate verbally with patients, their families and other health care professionals in the course of their work.

  • Support clinical supervision and clinical governance within the team.
  • Maintain a professional portfolio that demonstrates evidence of ongoing professional development.
  • Ensure practice is evidence-based through audit and critical appraisal of literature.
  • Maintain clinical standards through reflective practice and clinical supervision.
  • Adhere to the NMC Professional Code of Conduct.
  • Identify any limitations or gaps in professional or clinical knowledge.
  • Contribute to the development of clinical guidelines.
  • Act as a mentor for pre-registration nursing students.
  • Provide clinical education and training to patients and carers in the administration of intravenous antibiotic therapy under protocol.
  • Support the development of the nursing team by teaching and assessing junior members of the team.
  • Undertake mandatory in-house training and other training that supports the role.
  • Teaching of other health professionals to support patients in maintaining independence.

Person Specification

  • Essential Skills and Knowledge: IV skills, phlebotomy & cannulation skills; ability to communicate verbally and in writing at all levels; time management; problem solving; teaching ability.
  • Desirable: Care and management of differing types of vascular access (PICC, LTCC, Ports).

Experience

  • Essential: Post-registration experience; ability to facilitate other team members in providing IV Therapy; work autonomously.
  • Desirable: Community Experience.

Qualifications

  • Essential: RGN/RN; Teaching or Training Experience.
  • Desirable: Degree/Diploma level or willingness to work towards; modules such as Ethical & Moral Issues, Infection Control, Law & the Practitioner.

  • Essential: Committed to developing the service; the ability to travel independently across the Trust; the ability to work flexibly in accordance with service needs.
