RESPONSIBLE TO: Live-In Manager
MEASUREMENTS: Quality of service provided, measured through supervision, appraisal and Client and Carers/Support Workers feedback against the SweetTree Charter.
PURPOSE OF POST: To co-ordinate the work of Live-In Support Workers to provide high quality care and support to Live-in Clients to remain independent in their own homes, reflecting the objectives of the SweetTree Charter.
MAIN RESPONSIBILITIES:
- The job involves a high level of telephone contact with Live-In Clients and Carers/Support Workers to match the skills and experience of Carers/Support Workers to the individual needs of Client, whilst supporting the Live-in team in their roles to help achieve their objectives.
WORKING CONDITIONS: Working hours will be 37.5 weekly (On a rota basis: Tuesday- Saturday shifts between 8am- 8pm). Terms and conditions of employment are as per employment contract and staff handbook.
PRINCIPAL DUTIES:
- To ensure telephones are answered promptly and that all callers are spoken to in a polite and respectful manner.
- To maintain and upload accurate bookings and shifts onto OneTouch and the client profile, ensuring all information is correct, monitoring log-ins, and highlighting any errors or discrepancies to the relevant manager or team.
- To anticipate clients support requirements at least 4 weeks in advance and issue weekly rotas to the relevant Carers/Support Workers.
- To ensure schedules on OneTouch are accurate, and that any changes required for the generation of bookings are identified and flagged promptly to the relevant manager.
- To liaise with clients and their families regarding scheduling, ensuring any changes to team members or times are communicated efficiently and appropriately.
- To ensure Carers/Support Workers are appropriately matched to clients preferences and requirements, while promoting equal opportunities and respecting diversity, cultures and values, including preparing and sharing Support Worker profiles/bios with clients and families.
- To provide Carers/Support Workers with all relevant information and documentation prior to commencing a shift.
- To support the Live-in Service with the arrangement and facilitation of Support Worker introductions, attending where required to promote positive outcomes, and ensuring smooth transitions of Support Worker changes such as handover or sickness to support continuity of care.
- To liaise appropriately with professionals and family members forming part of the clients multi-disciplinary support team.
- To undertake regular well-being check-ins and high-quality support calls with Carers/Support Workers, updating OneTouch with accurate records of conversations.
- To support the Live-in Manager with Support Worker spot checks as required.
- To support the Live-in team with Support Worker facilitation's as designated by the Live-in Manager where appropriate.
- To support the Live-in team with monthly client calls and other client-related communications as designated by the Live-in Manager.
- To support with the training, development and upskilling of Carers/Support Workers, including participation in interviews where required.
- To provide support to the wider coordination team as and when required.
- To participate in the On-Call rota as required.
- To ensure the independence and dignity of clients is maintained at all times.
- To deliver an outstanding service to Carers and clients, maintaining SweetTrees reputation as a premier provider of care.
- To contribute to the development of the service and promote a positive image of clients and SweetTree Home Care Services.
- To participate in internal and external training, seminars and development opportunities.
- To prepare for and actively engage in regular supervisions, appraisals and performance reviews, both giving and receiving constructive feedback.
- To report, as a matter of urgency, any deterioration in physical or mental health, or environmental safety concerns, to the Manager or Registered Manager.
- Provide emergency shift cover when required.
- To assist in planning and organising travel arrangements, including bank holidays and Christmas Day.
- To attend and contribute to SweetTree meetings.
- To maintain confidentiality and adhere to data protection guidelines.
- To work in accordance with the Health & Safety at Work Act 1974.
- Any other duties as required.
PERSON SPECIFICATION
Essential experience, skills or attitudes:
- An outgoing, approachable and professional manner with a positive attitude.
- Proven ability to build and maintain positive working relationships with clients, families and Support Workers.
- Good understanding of how health and social care services are structured, particularly within community or live-in care.
- Awareness of the challenges faced by live-in Support Workers and Carers, with the ability to provide appropriate support.
- Experience of care within a community or live-in setting is desirable.
- Ability to work independently using initiative, as well as part of a team.
- Strong organisational, planning and time management skills with good interpersonal abilities.
- Reliable and consistent timekeeping.
- Strong administrative skills with good attention to detail.
- Good working knowledge of MS Office packages (Word, Excel, Outlook; Access desirable).
- Previous Care Coordination or similar experience in a fast-paced care environment is desirable.
- Willingness to support operational and hands-on tasks within a live-in service as required.
