Job overview
An exciting opportunity has arisen within the Urgent Community Response Directorate to recruit two Community Case Managers within the Virtual Ward Team.
We are seeking to appoint a dynamic, driven, motivated and caring individual who will bring their experience and personality to the role to ensure that as a team we align to the Trusts People, Patient, Population Strategy.
The role would suit someone with recent Emergency and Urgent Care experience within a Community setting.
Our current Virtual Ward Team consists of a range of multidisciplinary colleagues including:
- Occupational Therapists
- Physiotherapists
- Community Matrons
- Operational Managers
We currently have 6 Virtual Wards within the organisation including our award-winning Hospital at Home Service (EPCENTRE) lead in collaboration with our Acute Medicine colleagues. Working with the team, you will support across 4 of the Virtual Wards as detailed below:

Qualifications
- Registered General / Adult Nurse.
- Qualified Nurse Prescriber or willing to study for.
- Hold a recognised teaching/assessing qualification.
- Evidence of continued professional development.
- Evidence of Post Graduate study
- Evidence of subject related study
Experience
- Post registration experience working in community or acute care group.
- Specialist expertise / knowledge with underpinning theory of long-term conditions
- Documented evidence of relevant KSF based Continuous Professional Development (CPD)
- Experience of working as part of a multi-disciplinary team
- Experience of contributing to the personal development of less experienced staff, students or assistants
- Ability to work across professional and organisational boundaries.
- Evidence of post graduate experience
- Involvement in audit, research and understanding quality issues
Leadership
- Able to speak, receive and issue instructions concerning patients without risk of misunderstandings in group and individual settings.
- Able to communicate effectively in face to face contact, telephone contact, e-mail contact and internal and external meetings.
- Able to carry out frequent, long periods of medium-intense concentration carrying out assessments, planning treatment programmes, supervising support workers and more junior staff and dealing with complex equipment
- Able to deal sensitively with patients with high levels of anxiety/aggression which may be due to dementia, sedation, limited mobility or who are unable to accept current clinical outcomes.
