About the Candidate

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
  • Confident, capable, and clinically safe.
  • Organised and proactive in managing workload.
  • Communicates well with both patients and colleagues.
  • Comfortable working in a busy, structured environment.
  • Takes pride in delivering a professional and compassionate service.

Main duties of the job

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person‑centred plans for patients with, or at risk of, a long‑term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • New patient and health checks.
