Position: Scheme Manager (Supported Living)
Location: Kingston Upon Hull (Various locations with expectations of travel)
Hours: Full time, 40 hours per week and shift cover may be required when on-call.

About you
Do you have previous experience working in care, managing a team and want to use these skills to develop further and take the lead? This is an opportunity for a pro-active Scheme Manager to join an evolving Care Company and manage our residential living scheme in Hull.
You will need:
- Previous care management experience, preferably within a supported living environment.
- Hold or be working towards a NVQ Level 5 in Health & Social Care or have a suitable alternative qualification.
- Have excellent interpersonal skills and the ability to create an inclusive and welcoming environment for all.
- Evidence that you can provide inspiring, motivational, clear, and supportive leadership.
- The ideal candidate will be able to drive due to the needs of the service.

About The Service
In Swanton Community Care - Hull, we provide personal care and support for adults with mental health, dementia, learning disability and physical disability support needs. We have two 6-bedroom group living homes, Huckleberry House and Westbridge House along with another 3 services in the community, right in the heart of Hull.
This is an opportunity for an enthusiastic and motivated individual who is ready for a challenging role working across two supported living schemes in Hull. The role involves supporting three individuals with learning and physical disabilities to move into new tenancies. Our properties are in close proximity to each other. We offer a diverse and person-centred service to meet the needs of each individual, providing both supported living within a scheme or in individual settings. We support a diverse set of individuals with varying diagnoses and work with the local authority to identify and meet the needs of the people we support.
