£46,142 - £48,226 per year. Applications are welcome for the position of Senior Practitioner in the Review and Reassessment Team (Temporary for 18 months). This is a great opportunity to develop your skills as a Senior Practitioner in the recently developed Review and Reassessment Team. Our Team works with adults aged 18+ who are in receipt of support services commissioned by the Local Authority. The work of the Team is wide, ranging from the completion of planned annual Statutory Reviews to full Reassessments following substantial changes in people's lives. The Team is supportive, offering excellent peer and management support.
About the role
Our team uses the strength-based Collaborative Communication Framework to help people and their families identify what matters to them, giving them their voice, choice, and control. Our goal is to empower and promote independence to individuals, building on their strength and meaningful personal outcomes. Senior Practitioners support the Manager and team by ensuring good standards of work that is person-centred, non-oppressive, in line with legislation and the Social Work Code of Practice. Senior Practitioners sign off assessments and care plans, carry out regular supervision with staff members, and encourage continued professional development.
We are looking for candidates who are motivated, enthusiastic, and committed to social services. You will be passionate about relationship-based social services and providing the best possible outcomes for individuals in our communities. The Team are agile workers. We have an office base at the Guildhall, Swansea but spend much of the time working in the Community.
