We\'re looking for compassionate, organised, and proactive professionals to carry out statutory annual reviews of non-complex cases. You\'ll work closely with individuals, carers, service providers, and colleagues to ensure that care and support plans are meeting personal outcomes effectively.
Whether you join the Review and Reassessment Team or the Learning Disabilities Team, your role will include:
- Managing a caseload of non-complex reviews within agreed timescales
- Visiting individuals in their homes or care settings to assess and review needs
- Collaborating with families and professionals to ensure care remains appropriate and outcome-focused
- Identifying changes in need and escalating concerns to your supervisor
