£39,513 – £43,693 per year. (Full time for 2 years) We are currently seeking for two enthusiastic, organised and proactive Project Managers to join the Connecting Care team. In this exciting and challenging role, the successful candidate will be responsible for the successful delivery of the Connecting Care project including implementation of the Mosaic system.
As the Connecting Care Project Manager, you will have a diverse remit working closely with colleagues across either Adult Services or Child and Family services to successfully implement the new Mosaic case management system and support the development of a shared care record within the West Glamorgan region.
You will be responsible for successfully coordinating, monitoring and delivering against agreed targets using your understanding and experience of project management methodology and your excellent engagement and communication skills with all stakeholders.
You will be extremely flexible, self-motivated, and as a proven innovator enjoy being challenged. You will be an excellent communicator and driven to help make the Connecting Care project the best it can be for Swansea.
