Swansea Bay University Health Board is seeking a Band 8a Specialist Pharmacist to support inpatient cancer services at Singleton Hospital. You will deliver expert pharmaceutical care for cancer inpatients, including those receiving systemic anti-cancer therapy, within a multidisciplinary team.
The post holder will undertake medicines reconciliation, clinical review, medicines optimisation, discharge planning and provide specialist medicines information.
