Our mission statement within the Neath Port Talbot Community Reablement Team is: To promote independence and improve the daily lives of patients and their families in the community, when they need us. We are offering an exciting opportunity for an enthusiastic and innovative occupational therapist to join our busy, dynamic and friendly team. There is a strong emphasis on multi-disciplinary working especially within service development and team learning. The team work together supporting people to stay at home and thereby reducing escalating needs for secondary or social care. The team includes occupational therapists, physiotherapists, dietitians, speech and language therapists, rehab technicians, support workers, social workers, community wellbeing officers, nurses, health care support workers and administration assistants.

Main duties of the job:

The post will be based in Integrated Health and Social Care teams in Cimla Health & Social Care Centre. The post will provide an opportunity, as part of a multidisciplinary team, to provide rehabilitation to patients and groups of patients in a number of settings, including reablement wards, patients' own home and care homes. The post holder will be required to support therapy input into the exciting new Home First initiative. This post will support people to return home from hospital as soon as they are clinically optimised, allowing them to recover in their own home. The post holder will need to be able to adopt a flexible approach to work and will also be expected to participate in 7 day working.

Detailed job description and main responsibilities:

Person specification:

  • Qualifications
  • Essential criteria: Degree in Occupational Therapy (or equivalent OT qualification) plus relevant postgraduate level training or experience.
  • Professional Registration with HCPC.
  • Desirable criteria: Membership of professional body BAOT.
  • Experience and Knowledge
  • Essential criteria: Experience of autonomous caseload management, comprising assessment, planning intervention and evaluation within relevant clinical area.
  • Understanding of the Occupational Therapy process with particular emphasis on specialist area.
  • Broad knowledge of current best practice in Occupational Therapy with designated client group.
  • Knowledge of the principles of Clinical Governance and the ability to apply to working practice.
  • Desirable criteria: Experience of research and/or audit and implementing evidence-based practice.
  • Previous experience of supervising staff.
