Job Responsibility:
An exciting opportunity has arisen for 2 motivated and enthusiastic people to join Area 1&2 Secondary Care Team as a Community Support Worker. The Team provides monitoring and support to older people with enduring and complex mental health needs. The post holder will be required to work as part of a team providing support to patients in receipt of Secondary Care Mental Health Services.
Key responsibilities include:
- Assisting individuals to achieve and maintain optimum levels of independence by providing support, choice and dignity in their daily living via assessment, implementation and evaluation of agreed plans of care.
- Working alongside the Registered Nurse in providing and monitoring an enhanced level of support, assessment and treatment to patients under Secondary mental health Care in the community.
- Working collaboratively with the Multi-disciplinary team (MDT) to ensure the maximum benefits to patients/residents using a patient centred approach to care.
- Observing and monitoring medication side effects, physical observations and mental health of patients and reporting these to the MDT.
