Swansea Bay University Health Board is seeking a Transitional Liaison Mental Health Practitioner to support high-risk OPDP-eligible female offenders transit safely from prison and Approved Premises to the community. The role forms part of the developing Welsh Intensive, Intervention and Risk Management Service (IIRMS). You will work to improve access to services, coordinate between England and Wales facilities, and collaborate with statutory and voluntary agencies to meet complex needs and risk.
