Type: Full-time or Part-time (Flexible hours)
Availability: Monday to Friday 08.00-18.30, Saturday availability beneficial, with the possibility of working on Mondays till 8pm.
Job Overview:
As a Practice Nurse at Swanley Medical Practice, you will be responsible for a number of clinical areas such as health promotion, chronic disease management, and illness prevention. You will contribute to the smooth running of appointments, improving patients' experience in the surgery.
Main duties of the job:
- Responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team.
- Identifying, managing, and supporting patients at risk of developing long-term conditions to prevent adverse effects on their health.
- Routine nursing care according to clinical-based evidence, NICE, and the NSF guidelines.
- Providing vaccinations/immunisations and travel medicine, wound care, requesting necessary pathology services with subsequent result processing.
- Conducting chronic disease clinics, delivering patient care and making referrals to secondary/specialist care when required.
- Prioritising health issues, recognising, and referring patients with mental health needs, and supporting patients in medication use.
- Safeguarding matters, delivering opportunistic health promotion and the management of emergency drug kits.
Health Promotion:
- Support health promotion initiatives within the practice by providing information to patients on healthy lifestyle choices and encouraging patients to engage in preventive healthcare measures.
- Contribute to patient education programs within the practice.
Infection Control:
- Adhere to infection control protocols to maintain a safe and clean environment for patients and staff.
- Assist in maintaining the cleanliness and organization of clinical areas.
Collaborate with healthcare professionals, including GPs, nurses, and administrative staff, to ensure smooth operations within the practice.

With a patient list of 24,000, we boast a dedicated team, including GP Partners, Salaried GPs, Clinical Pharmacists, four Advanced Nurse Practitioners, Nurses, HCAs, and other clinicians from our Primary Care Network. Notably, we are also a GP Training Practice.
