We have an exciting and rare opportunity for a Lead Practice Nurse to join our team at Swanage Medical Practice. This is a key role for a passionate nurse ready to lead our nursing services and provide clinical care. You will be instrumental in delivering the highest standards of clinical care, developing patient services and supporting our Practice nursing team.

Main duties of the job

  • Clinically lead on the organisation and co‑ordination of the provision of nursing services for the practice.
  • Provide practice nursing care to include wound care, cervical smears, ECG, venepuncture, vaccination and immunisations and travel vaccines.
  • Support and treat patients with asthma and create individual asthma treatment plans.
  • Be competent in managing patients with complex leg ulcers.
  • Support the duty team during an emergency medical situation.
  • Act as a clinical leader in the delivery of practice nursing services to patients.
  • Support the development of others to make realistic self‑assessment of their knowledge and skills, challenging any complacency and actions that are not in the interest of patients.
  • Monitor work areas and practices to ensure they are safe and free from hazards and conform to health and safety, security legislation, policies and procedures.
  • Act as the designated lead for Infection prevention and Control, overseeing audits, training and policies.
  • Ensure all treatment‑room staff have annual appraisals and where necessary performance reviews.
  • Participate in practice team meetings, delivering the nursing agenda and run nurse department meetings.

Job responsibilities

  • Assist with the organisation and co‑ordination of the provision of nursing services for the practice.

  • Support and treat patients with asthma and create individual asthma treatment plans.
  • Be competent in managing patients with complex leg ulcers.
  • Support the duty team during an emergency medical situation.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self‑care.
  • Chaperone when necessary.
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Deliver care according to NSF, NICE guidelines and evidence‑based care.
  • Evaluate patients' response to health care provision and the effectiveness of care.
  • Support and participate in shared learning across the practice and wider organisation.
  • Delegate appropriately, adopting the principles of safe practice and assessment of competence of nurses and HCAs.
  • Contribute nursing views to Practice management team.
  • Monitor the performance of the nursing team in accordance with local policies.
  • Ensure robust systems are in place to monitor safety and quality such as the following audits: spot check cleaning, cold chain, handwashing, infection control, clinical waste and any other necessary audits.
  • Ensure robust systems are in place to check treatment room stock and processes are in place to monitor manufacturers expiry dates.
  • Ensure robust processes are in place to manage all Patient Group Directives (PGDs).
  • Lead on the organisation of treatment room services for house‑bound patients such as vaccinations and immunisations.
  • Lead on the introduction of new nursing services such as new vaccinations and immunisations introduced by Public Health.
  • Work as an effective and responsible team leader, supporting others and exploring the mechanisms to develop new ways of working and ensure planning and implementation of local policies, projects and National standards.

Administrative and professional responsibilities

  • Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model.
  • Ensure accurate clinical records and use of SNOMED CT codes at all times.
  • Contribute to the Practice QOF targets and Enhanced Services.
  • Contribute to the development and review of local guidelines, policies and standards.
  • Prioritise own workload and ensure effective time‑management strategies are embedded within the culture of the team.
  • Discuss, highlight and work with the team to create opportunities to improve patient care.
  • Promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice and participate in the education of students.

We have an experienced and excellent team of Practice Nurses, HCAs, Phlebotomists and admin team. We are supported by the Purbeck Primary Care Network team, which includes First Contact Physiotherapists, Clinical Pharmacists, Mental Health Practitioners, Respiratory and Diabetes specialist support and Care Coordinators. We are a training Practice and have a buddy system in place to help with continued support and learning and we offer ongoing training and CPD opportunities. Experience

  • Broad knowledge of general practice and experience of working as a Practice or Community Nurse, including wound care, ECG, venepuncture, immunisations.
  • Experience of Infection Prevention and Control Measures.
  • Experience of leading a team.
  • Experience with the management of Asthma.
  • Understanding the importance of evidence‑based practice.
  • Experienced with IT such as Word, emails, Internet.
  • Knowledge of SystmOne.
  • Experience with the management of complex leg ulcers.
  • Experience of PGDs and associate policies.
  • Experience of reviewing and updating policies.
  • Knowledge of clinical audits.

Qualifications

  • Professional registration with the NMC.
  • Accredited training in Asthma.
  • Cervical Cytology qualification.
  • Mentor or teaching qualification.
