This role offers a local GP the opportunity to join a supportive Primary Care Network, helping them build confidence and develop skills beyond traditional general practice. The GP will work under the Additional Roles Reimbursement Scheme (ARRS), supporting the delivery of PCN DES Enhanced Health in Care Homes. The role will focus on providing proactive person-centred medical care for our care home residents, housebound or frail patients within the SWAN PCN footprint including our aligned care homes and working closely with our multidisciplinary team. The role will be based in SWAN PCN practices and the care homes locally that are aligned to the PCN.

Main Responsibilities

  • Provide face-to-face and telephone consultations for routine and urgent care for patients in local care homes and for patients with frailty.
  • Conduct patient care home visits and ward rounds.
  • Manage patient care, including investigating and reviewing results, developing care plans, and making referrals to community, voluntary and secondary care or other providers.
  • Issue and sign repeat prescriptions, ensuring they align with national/local guidelines.
  • Administer and handle patient-related queries, test results, referral letters, and other tasks in a timely manner.
  • Record clear, contemporaneous consultation notes into S1 clinical system.
  • Ensure safe, evidence-based, and cost-effective care, in line with Practice/PCN disease management protocols and NICE guidance.
  • Independently prescribe medications within your scope of practice.
  • Review and manage patient medications following appropriate policies and local clinical guidelines.
  • Utilise other available clinical and support staff in delivering holistic and patient-centred care.
  • Actively participate in MDT and care conference meetings, as required.
  • Contribute to the delivery of multi-disciplinary proactive care for complex patients at greatest risk of deterioration and hospital admission.

  • Regularly screen for lifestyle factors and support patients in adopting health promotion strategies and self-care principles.
  • Contribute to the delivery of anticipatory care plans.

  • Maintain awareness of clinical practice developments.
  • Attend PCN meetings, education training and clinical meetings.
  • Participate in an annual performance review and take responsibility for personal development.
  • Engage in mandatory training and attend courses as needed.
  • Provide clinical support and guidance to both clinical and administrative staff to maintain the highest care standards.
  • Work closely with practices from SWAN PCN.
  • Collaborate with health and social care providers to deliver an integrated, safe service.

Risk Management and Compliance

  • Adhere to all practice policies and procedures, including those related to prescribing, confidentiality, and data protection.
  • Apply infection control measures and manage risks in compliance with local and national guidelines.
  • Contribute to clinical governance through audits, peer reviews, and significant event analysis.

Patient Safety and Confidentiality

  • Ensure patient and carer confidentiality, in line with practice policies.
  • Manage patient data securely, only disclosing it to authorised persons.

Teaching & Wider Development

  • Participate in teaching or supervision of students where appropriate.

Person Specification

  • Must be fully registered with the General Medical Council.
  • Excellent written and verbal communication skills with the ability to make confident decisions.
  • Ability to work within professional codes of conduct, ensuring safe and effective patient care.
  • Ability to manage time, workload, and patient-related administration efficiently.
  • Willingness to work across multiple practices and collaborate with multidisciplinary teams.
  • Experience with SystmOne preferred.
  • Experience with GM Care record preferred.
