We are seeking a proactive and compassionate Care Coordinator to join Swaffham and Downham Market (PCN). The post holder will support patients with personalised care planning, coordinate services, and promote continuity of care within the PCN. The role requires close working with multidisciplinary teams, patients, carers, and other professionals to improve patient experience and outcomes

Swaffham and Downham Market PCN is based in West Norfolk, our PCN serves 46,413 (NHS England, 2024) patients across our seven member practices.We pride ourselves in offering our patients the best care possible, demonstrating innovation, clinical excellence and working collaboratively with our allied health care professionals across the network. This position will be based at Manor Farm Centre Swaffham, although candidates may be asked to work at their branch site in Necton.

Position is part time 25 hours

Main duties of the job

  • Proactively work wit patients to support their personalised care requirements.
  • Provide coordination and navigation support using digital tools to help patients and their carers access appropriate health and care services.
  • Support patients in managing their conditions, answering queries, and navigating appointments.
  • Ensure that patients have access to high-quality information to support informed choices about their care.
  • Promote preventative healthcare and continuity of care.
  • Support in administrative duties across the practice to support the patient and practice need.

About us

Manor Farm Medical Centre is a multi-disciplinary practice hosting numerous clinics.

It was converted from a barn in 2001 and offers a pleasant and welcoming atmosphere in a combined traditional and modern setting.The practice is situated close to the centre of Swaffham and also has a branch site in Necton, called Oak Farm.

Job responsibilities

This is an administrative role that may encompass some of the following tasks, however other tasks may be required by the practice as this role embeds within the scope of the position.

Patient-Focused Care Coordination:

  • Proactively work with patients to support their personalised care requirements.
  • Provide coordination and navigation support using digital tools to help patients and their carers access appropriate health and care services.
  • Support patients in managing their conditions, answering queries, and navigating appointments.
  • Ensure that patients have access to high-quality information to support informed choices about their care.
  • Promote preventative healthcare and continuity of care.

Operational & Administrative Duties:

  • Provide administrative and operational support to the practice and PCN.
  • Process referrals, make appointments, and ensure appropriate triaging to relevant team members.
  • Maintain accurate and up-to-date records on EMIS and other relevant systems.
  • Liaise with internal and external stakeholders, including patients, families, clinicians, and allied professionals to ensure effective service delivery.
  • Respond to enquiries in a professional, welcoming, and person-centred manner.
  • Support the administrative planning and delivery of flu and COVID-19 vaccination campaigns.
  • Provide admin support to vaccinators as required.
  • Patient/letter coding within the clinical systems
  • Support efforts to reduce health inequalities by adopting a population health management approach.
  • Contribute to the delivery of Core20PLUS5 priorities.

Person Specification

Experience

  • Excellent communication and interpersonal skills
  • Strong organisational and administrative abilities
  • Ability to work independently and as part of a team
  • Experience working in Primary Care or across multidisciplinary teams
  • Trained in the use of EMIS or S1
  • Understanding of population health and health inequalities strategies
