Salaried Gp - Pcn & Home Visiting Service

This is an exciting opportunity for an experienced GP to join Swadlincote PCN in a varied and rewarding role supporting our PCN and Home Visiting Service. The successful candidate will provide high-quality, patient-centred care for people with complex needs, work closely with a skilled multidisciplinary team, and help shape services that make a real difference to our local community. The post holder will provide high-quality, patient-centred clinical care with a particular focus on home visiting, frailty, admission avoidance and supporting patients with complex or uncertain presentations. Working autonomously and as part of a multidisciplinary team, the Salaried GP will assess, diagnose, manage and review patients in the community, making safe clinical decisions and escalating care appropriately where needed.

The role will also involve providing clinical support and guidance to colleagues within the PCN team, including advanced practitioners, paramedics and other healthcare professionals. The successful candidate will contribute to multidisciplinary working, clinical governance, service development and quality improvement, helping to strengthen community-based care and ensure patients receive the right care in the right place at the right time.

Our PCN premises is located in a rural South Derbyshire village close to Swadlincote. It is within easy reach of Burton-Upon-Trent, Derby, Lichfield, Leicester and the M42. Swadlincote is a rural market town with country-side charm, good schools and a strong community spirit, making it a great place to live and work.

We are offering flexible session of up to 4 sessions per week with a competitive salary and NHS pension.

Main duties of the job:

• Work with the PCN Clinical Directors, PCN Manager, Team Up Operational Lead and other senior clinical leads within the PCN to develop and implement new care pathways for the service areas of acute home visiting, urgent community response, enhanced health in care homes, anticipatory care and other PCN led clinical services that supports the multi-agency and multi skilled team approach.
• Developing clinical leadership is an important part of our team culture. The successful applicant will have opportunities to build confidence in leadership through multidisciplinary working, supporting colleagues, contributing to governance and quality improvement, and becoming involved in service development.
• As part of the role, you will also provide clinical support to the PCN Team working in Ragsdale House. The team is made up of ANPs, Paramedics, Social Prescribers, FCPs, Mental Health Practitioners and Health Care Support Workers.
• Support the teams in identifying and responding to safety and quality concerns, ensuring these are escalated as appropriate.
• Providing medical assessment and management where needed on behalf of, or in conjunction with, team members.
• Work with the Team Up operational lead and Home Visiting Service clinical lead(s) to develop and implement (with appropriate clinical protocol) the senior clinical triage function for the Home Visiting element of the service to determine the urgency and type of response according to clinical need.

Person Specification:

Essential:

• Disclosure and Barring Service Check Certificate
• Experience of working autonomously as a GP.
• Experience of assessing and managing acute, complex and uncertain clinical presentations.
• Experience of making safe clinical decisions where information may initially be limited.
• Experience of working collaboratively with other healthcare professionals.
• Experience of managing clinical risk and recognising when escalation is required.
• An interest in developing clinical leadership skills.

Desirable:

• Experience of home visiting or community-based clinical care.
• Experience of urgent care, frailty, care-home medicine or admission avoidance.
• Experience of clinical triage.
• Experience of supporting advanced clinical practitioners, paramedics or other multidisciplinary clinicians.
• Experience of multidisciplinary working across organisational boundaries.
• Experience of leadership, education, supervision, quality improvement or service development.

Skills and Attributes:

• Excellent communication and interpersonal skills.
• Proven ability to offer support within a clinical team and share learning points in multi-professional teams.

Qualifications:

• Full registration with the General Medical Council.
• Current licence to practise.
• Inclusion on the National Medical Performers List.
• Certificate of Completion of Training in General Practice, or an equivalent recognised qualification.
• At least two years post-CCT experience in general practice.
• Current UK professional registration.
• Eligibility to work in the United Kingdom.
