We are currently recruiting for an experienced Social Worker / Approved Mental Health Professional (AMHP) to join the Older People's Mental Health Team based at Springfield Hospital, Tooting, working with the London Borough of Merton.

This is an excellent opportunity to become part of a supportive multidisciplinary team, making a real difference to older adults experiencing complex mental health needs.

Key Responsibilities

  • Manage a complex caseload within the Older People's Mental Health Team.
  • Complete Care Act assessments, reviews, and person-centred care planning.
  • Undertake safeguarding enquiries and comprehensive risk assessments.
  • Work within the Care Programme Approach (CPA) framework.
  • Contribute to Mental Health Tribunals and Section 117 aftercare planning.
  • Work collaboratively with healthcare professionals, service users, carers, and families.
  • Support individuals to maintain and develop their social networks, independence, and community inclusion.
  • Participate in the AMHP duty rota if AMHP qualified.

Requirements

  • Qualified Social Worker with Social Work England registration.
  • Experience working within adult mental health services.
  • Strong knowledge of the Care Act, Mental Health Act, Mental Capacity Act, and safeguarding legislation.
  • Experience working within multidisciplinary teams.
  • AMHP qualification is desirable, with the opportunity to participate in the duty rota if qualified.
