SW Locums is recruiting an experienced Senior Qualified Social Worker (Health Liaison Worker) – Adults for a full-time locum role with the London Borough of Merton.

  • £36.50 per hour
  • 35 hours per week
  • Monday to Friday

  • Full-time locum

About the Role

You will work closely with community health colleagues, particularly GPs, to strengthen integrated health and social care working. The role focuses on targeted case management, promoting independence and self-management, helping people remain in their own homes, and preventing unnecessary hospital and care home admissions. You will also support the day-to-day running of care management activities, provide cover for the Team Manager/Assistant Team Manager when required, and carry out statutory duties.
