The Role
We are looking for an experienced and confident Senior Practitioner to support Adult Social Care services within a fast-paced environment. The successful candidate will:
- Support and supervise Hospital Social Workers.
- Provide strong professional guidance and support to colleagues.
- Work effectively within a multidisciplinary team (MDT) in a fast-paced setting.
- Have experience working within Hospital and Acute settings.
- Work closely with Reablement services to support positive outcomes for adults.
- Contribute to effective assessments, care planning and safe discharge processes.
- Manage complex situations and provide professional oversight where required.
- Work collaboratively with health and social care professionals to achieve the best outcomes for service users.
The role is particularly suited to a Senior Social Worker or Senior Practitioner with strong hospital, acute, reablement experience.
