This is an exciting opportunity to work at the front door of Adult Social Care within a fast-paced, integrated health and social care team.

You'll play a key role in delivering timely interventions and supporting vulnerable adults when they need it most.

Key Responsibilities

  • Complete Care Act and Mental Capacity Assessments.
  • Manage telephone triage, duty responsibilities, and urgent referrals.
  • Undertake safeguarding enquiries and dynamic risk assessments.
  • Respond effectively to crisis situations.
  • Work collaboratively with Social Workers, Occupational Therapists, Nurses, Physiotherapists, and Social Care Assistants to achieve positive outcomes.

Requirements

  • Qualified Social Worker with Social Work England registration.
  • Adult Social Care experience is essential.
  • ASYE completed is desirable.
  • Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation.
  • Excellent communication skills with the ability to thrive in a busy, multidisciplinary team.
