This is an exciting opportunity to work at the front door of Adult Social Care within a fast-paced, integrated health and social care team.
You'll play a key role in delivering timely interventions and supporting vulnerable adults when they need it most.
Key Responsibilities
- Complete Care Act and Mental Capacity Assessments.
- Manage telephone triage, duty responsibilities, and urgent referrals.
- Undertake safeguarding enquiries and dynamic risk assessments.
- Respond effectively to crisis situations.
- Work collaboratively with Social Workers, Occupational Therapists, Nurses, Physiotherapists, and Social Care Assistants to achieve positive outcomes.
Requirements
- Qualified Social Worker with Social Work England registration.
- Adult Social Care experience is essential.
- ASYE completed is desirable.
- Strong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislation.
- Excellent communication skills with the ability to thrive in a busy, multidisciplinary team.
