This post of PCN Lead Nurse is for an experienced and qualified GP Practice Nurse who has senior or advanced practice qualifications including clinical and leadership skills.

Provide leadership and management support to the Nursing and HCA teams within the PCNs, including participation in the annual appraisal and performance management processes.

Liaise with the Sutton PCNs Executive to recruit, develop and manage the Nursing and HCA team.

Collaborate with the Clinical Governance Quality and Safety (CGQS) Committee to manage patient expectation, service quality and organisational safety in line with CQC requirements.

Lead and oversee Infection Prevention and Control in Patient Services, managing outsourcing provision as appropriate.

Ensure that the Nursing and HCA team work collaboratively with other members of the PCN team across the four PCNs to enable effective synergy.

Work in accordance with the Nursing and Midwifery Code of Conduct (NMC).

Work 1 evening 4-8pm or Saturday mornings as required.

Chair meetings as required, including but not limited to nurse team meetings, medication safety and IPC reviews.

Sutton has a population of approximately 200,000 residents registered to 21 practices and there are currently 4 Primary Care Networks each serving a population of approximately 50,000 patients.

Our Sutton PCNs are forward-looking, friendly, and focused on providing a wide range of excellent healthcare services to patients in Sutton and the surrounding area.

Our PCNs between them are led by 8 PCN Clinical Directors. The PCNs work together as they see the benefits of working together in a larger GP partnership and are delighted to be realising some of those benefits now.
