To deliver NHS Health Checks to eligible Sutton patients within Enhanced Access clinics, undertaking clinical assessments, blood tests, accurate documentation, patient education, and follow-up in line with NHS Health Check guidance, Sutton PCN protocols, and NICE guidelines.
Main duties of the job:
- Deliver NHS Health Checks to eligible patients in line with the NHS health check programme.
- Undertake face-to-face assessments, including:
- Height, weight, BMI and waist circumference
- Blood pressure measurement
- Lifestyle assessment (smoking, alcohol, physical activity, diet, QRISK, QDiabetes)
- Blood test (e.g. cholesterol, HbA1c)
Experience:
- Working as a Health Care Assistant in primary care settings.
- Confident with patients from diverse backgrounds.
- Basic IT skills and experience using GP clinical system EMIS.
- Ability to work independently and manage appointment time effectively.
- Experience of working across multiple GP practices.
Qualifications:
- NVQ Level 3 in Health & Social Care (or equivalent experience).
- Knowledge of local lifestyle/prevention services.
- Enhanced DBS required.
- Mandatory training must be up to date (safeguarding, infection control, IG etc).
- Uniform or dress code in line with PCN policy.
Sessions including evenings and weekends.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
