Deliver NHS Health Checks to eligible patients in line with the NHS health check programme.
Undertake face-to-face assessments, including:
- Height, weight, BMI and waist circumference
- Blood pressure measurement
- Lifestyle assessment (smoking, alcohol, physical activity, diet, QRISK, QDiabetes)
- Blood test (e.g. cholesterol, HbA1c)
Complete follow-up telephone appointments.
Accurately record all activity on the clinical system - EMIS.
Calculate and explain CVD risk scores in a clear and supportive way.
Provide brief advice and signposting to lifestyle services.
Escalate or refer patients appropriately in line with PCN protocols.
Ensure informed consent, confidentiality, and data quality standards are met.
Work across multiple practices as required by the PCN.
Adhere to infection prevention, safeguarding, and information governance requirements.
Participate in Health Care Assistant meetings to support development.
Identify abnormal results (e.g. raised blood pressure, cholesterol, HbA1c) and escalate appropriately in line with local protocols.
Maintain infection control standards and safe handling of specimens.
Use emergency equipment and medications appropriately and escalate concerns promptly.
Report incidents, learning events, safeguarding concerns, and accidents in line with policy.
Protect patient confidentiality and follow information governance requirements.
Experience working as a Health Care Assistant in primary care settings.
Competent in delivering NHS Health Checks.
Confident with patients from diverse backgrounds.
Basic IT skills and experience using GP clinical system EMIS.
Ability to work independently and manage appointment time effectively.
Experience of working across multiple GP practices.
