Sutton Primary Care Networks is recruiting a Patient Contact Coordinator to support the Proactive Anticipatory Care (PAC) Service. You will proactively contact identified patients, explain aims and benefits, address questions and encourage participation in proactive healthcare and wellbeing support.
In this role you will handle telephone enquiries, maintain accurate records, arrange appointments, and work with the multidisciplinary team to improve patient uptake and accessibility of the service.
