Main Duties, Tasks & Skills Required
- To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.
- To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.
- To undertake ongoing assessment, treatment and evaluation for service users known to the service.
- To support the delivery of therapeutic interventions either individually or within a group setting.
- Support the transformation of the team to deliver local evidence-based care.
