Are you interested in becoming part of an innovative community Older Persons Mental Health Service, which focuses on all areas of mental health, irrespective of functional, organic or complex illness on the Isle of Wight? We are aligning our focused OPMH pathways with all age Community Mental Health Services and are looking for passionate, highly motivated individuals to work as part of the Older Persons Mental Health Service. The post holders will undertake the role of care co‐ordinator / care manager in the assessment, treatment and delivery of care to older people with mental health problems who meet the criteria for access to the service. They will be responsible for ensuring that the highest quality care is carried out in accordance with policies and procedures of Hampshire and Isle of Wight NHS Foundation Trust.

You will need to be a Registered Mental Health Nurse (RMN), Occupational Therapist (OT) or Social Worker and have experience of working with people with serious and enduring mental health needs. The ability to work in partnership with carers/family members and services in the community is essential.

Main duties of the job:
  • To undertake the role of care co‐ordinator / care manager in the assessment, treatment and delivery of care to older people with mental health problems who meet the criteria for access to the service.
  • The post holder will ensure that the highest quality care is carried out in accordance with policies and procedures of Hampshire and Isle of Wight NHS Foundation Trust.

Person Specification:
  • Qualifications: RN [MH] with live NMC registration or OT or Social worker with current professional registration. Nurse Mentorship Qualification or relevant professional equivalent.
  • Experience: Evidence of ongoing development and post-registration training. Post-registration training in evidence-based psychological treatments. Training in Risk assessment and management skills. Ability to build on the knowledge, skills and priorities of people working in a range of disciplines. Experience in the assessment and management of people with serious and enduring mental health. Evidence of undertaking audit work. Innovative, creative and a willingness to try new ideas.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary: £38,682 to £46,580 a year based on full-time hours.
