Following a promotion within the service, an opportunity has arisen for Band 7 Nurse/Paramedic within the Urgent Community Response Team (UCR). In this role, you will deliver a rapid 2-hour response to patients experiencing a health crisis, aiming to prevent unnecessary hospital admissions and support care in the community as part of the Virtual Ward core clinical delivery team.

The UCR service is an exciting and rapidly evolving part of the NHS, playing a pivotal role in delivering acute care within the community. Aligned with the NHS Long Term Plan's focus on enhancing community-based services, UCR is at the forefront of clinical innovation and integrated care. Recent pilot collaborations with the ambulance service have demonstrated the critical impact our staff have in improving patient outcomes and reducing hospital admissions. These initiatives highlight the essential role UCR plays in shaping the future of urgent care, where service development is fast‑paced, dynamic, and deeply patient‑centred.

Main duties of the job

  • Deliver short‑term, intensive case management to patients in crisis who are at high risk of unplanned hospital admission, typically for up to 72 hours.
  • Respond promptly to patients requiring urgent assessment, working collaboratively with patients and their support networks to develop management plans that enable safe care at home.
  • Support ambulatory care pathways for conditions such as UTIs, pneumonia, and cellulitis, including the initiation and administration of IV therapies.
  • Work closely with Virtual Ward colleagues and other members of the community integrated team to ensure seamless care and continuity for patients.
  • Play a key role in preventing unnecessary hospital admissions by delivering timely, patient‑centred interventions and coordinating care with wider community services.

Job responsibilities

  • Act as a senior clinician within the UCR team, offering guidance, mentorship, and support to junior staff and contributing to a culture of excellence and continuous learning.
  • Undertake line management duties including supervision, appraisal, and professional development of team members.
  • Lead and contribute to quality improvement initiatives, using data and feedback to inform service development and enhance patient outcomes.
  • Play an active role in the ongoing integration of UCR with other services, including the Virtual Ward and Integrated Night Service, helping to shape a responsive and cohesive 24‑hour care model.
  • Promote evidence‑based practice and contribute to the development of clinical guidelines, pathways, and protocols that support safe, effective, and person‑centred care.

Person Specification

  • Able to demonstrate effective leadership, communication and organisational skills.
  • Advanced clinical decision‑making skills to support patient centred care.
  • Advanced clinical skills ie cannulation, administration of IV medication.
  • Advanced physical assessment skills or willingness to undertake within 12 months.
  • Ability to influence and manage a variety of change management programmes in embracing and implementing change to provide high quality health outcomes.

Experience

  • Experience of working as an autonomous practitioner in either an acute or community environment.
  • Multi‑disciplinary and multi agency working.
  • Proven extensive experience of working with adults/older people.
  • Experience in a Community setting, or gateway ability to interpret blood results.
  • Experience of working between other health and social care organisations/settings.

Qualifications

  • Degree/diploma in paramedic science.
  • HCPC registration/NMC registration.
  • Teaching and Assessing qualification.
  • Non medical independent prescriber / supplementary prescribing, or willingness to undertake training within 12 months of coming into post.
  • Evidence of continuing professional development in a relevant area of specialist practice to Masters equivalent level.
  • Community Nursing Qualification / Advanced Nurse Practitioner Qualification.
  • Trained to interpret microbiology and pathology results.
  • Case Management.
  • Advanced physical assessment skills.
  • IRMER trained.
