The post holder is a Nursing Associate registered with the Nursing and Midwifery Council.

Thepost holder will work as part of the general practice nursing team, providingnursing care to patients registered with the GP practice(s) within the PCN. The post holder will hold their own clinicalcaseload to support the needs of patients registered with the Practice(s) andto compliment the needs of the nursing team. This will include undertaking activities asoutlined in the current Network Contract and associated direct enhancedservices, such as supporting Personalised Care Support Planning and providingcardiovascular health checks.

Workingwith Multidisciplinary Teams, inclusive of General Practice Nurses and GPs youwill deliver collaborative care that focuses on high quality care adding impactand value to benefit patients, and the General Practice Workforce whereappropriate. This may include working collaboratively to organise andfacilitate care appropriate to the individual needs of the patient.

Main duties of the job

Tobe able to demonstrate evidenced based practice when undertaking care anddelivering Health checks which may be working with patient cohorts who presentwith i.e., high blood pressure, diabetes, obesity, epilepsy, respiratoryissues, constipation, lack of personal hygiene, including foot care.

To be able to demonstrate an ability to undertake baseline physicalobservations and record results correctly. In addition, be able to identifyanomalies that require further guidance and advice in reporting to a seniorclinical peer for escalation as a priority for intervention.

Person Specification

Experience

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary teams.
  • Understands and acts in line with NMC professional standards for practice contained within The Code.
  • Knowledge of when to seek advice and elevate to the appropriate professional for expert help and advice.
  • Understand requirements for NMC professional Revalidation.
  • Demonstrate ability to plan and develop care pathways to meet needs holistically for the patient.
  • Experience of providing and receiving complex, sensitive information.
  • Experience of working in General Practice.
  • Experience of managing patients in general practice and supporting patient with learning disabilities, severe mental illness, diabetes and those residing in Care Homes.

  • Ability to support, supervise, assess and act as a role model to nursing associate students , other learners and health care support workers within the SH PCN and clinical setting.

Qualifications

  • GCSE Maths and English (C and above) or equivalent.
  • Registered Nursing Associate on the NMC register.
  • Supervision, mentoring, teaching or assessing qualification.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Depending on experienceÂŁ27,456 per annum (pro-rata for part time)
