Sussex Primary Care Ltd is recruiting a Lead Nurse to join The Avenue Surgery team in Brighton. The role involves providing specialist nursing services within the GP practice, working with GPs, pharmacists, paramedics, practice staff and patients to deliver excellent primary care.

Job Summary

The Lead Nurse will act as a clinical leader, manage and motivate the nursing team, lead local projects and initiatives, support quality, infection prevention, safeguarding, and health improvement activities. The successful candidate will be responsible for meeting local and national targets and ensuring the highest standards of care for patients in the practice.

Responsibilities

  • Act as a clinical leader in the delivery of practice nursing services to patients, ensuring that the needs of the patient are the priority.
  • Lead the nursing team in the planning and implementation of local guidelines, protocols and standards, and of local projects or initiatives.
  • Promote the role of the nursing team in the provision of care.
  • Participate in practice/clinical meetings, delivering the nursing agenda, and running nurse department meetings.
  • Support QoF strategy and delivery for long‑term condition management, maximising income.
  • Take responsibility for managing and maximising ICB, PHE and NHSE‑driven targets (childhood immunisations, flu campaigns, smears etc).
  • Support the development of others in order to maximise staff potential.
  • Act as the infection control lead and complete IPC audits, develop action plans and ensure implementations are embedded.
  • Work closely with the ACP Clinical Lead.
  • Line‑manage the Practice Nurses, providing regular supervision and completing annual appraisals.
  • Promote continuous professional development and evidence‑based practice.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Deliver care as an individual and team according to NHS and NICE guidelines.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation.
  • Utilise the audit cycle to evaluate the quality of work and implement improvements where required.
  • Collaborate with other clinical teams to improve healthcare in response to local and national policies.
  • Explain and implement new ways of working to support patient care and staff wellbeing.
  • Provide educational role to patients, carers, families and colleagues in an environment that facilitates learning.
  • Identify patterns of discrimination and take action to promote diversity and equality of opportunity.
  • Enable others to promote equality and diversity in a non‑discriminatory culture.
  • Support people who need assistance in exercising their rights.
  • Act as a role model in the observance of equality and diversity good practice.
  • Accept the rights of individuals to choose their care providers, participate in care and refuse care.
  • Assist patients from marginalised groups to access quality care.
  • Undertake any required training to meet the needs of the practice.

Qualifications

Essential

  • Relevant 1st nursing degree/MSc
  • Mentor/teaching qualification
  • Evidence of continuous professional development

  • Independent Prescribing Course

Experience

  • Working autonomously in a senior nursing role within Primary Care
  • Experience of clinical audit
  • Experience in nurse‑led management of long‑term conditions
  • Experience in implementing protocols and clinical guidance
  • Team Leader/Line management experience
  • Teaching and mentoring experience in primary care

Desirable

  • Experience in Quality Improvement Initiatives
  • Community Nursing Experience
