We\’re seeking Band 6 Urgent Care Practitioners to join our Crisis Resolution and Home Treatment Team (CRHTT) and help develop a new and innovative Urgent Care Hub.
The Urgent Care Hub is a collaboration between our CRHTT and Haven team, working across the urgent care service at Sussex Partnership. These services are vital to our Urgent Care Pathway, ensuring that individuals presenting to acute mental health services receive compassionate, person-centered assessments and timely care plans, whether in The Haven or with CRHTT in their homes.
This role offers a £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners (pro rata, subject to terms and conditions; see attached Protocol Document for details).
This dynamic position provides opportunities for developing and implementing high-quality services. A typical day involves working with The Haven, the acute hospital, and community teams to support individuals in mental health crisis.
The successful candidate will demonstrate excellent communication skills, empathy, and the ability to foster an inclusive and collaborative environment. Your focus will be on assessing service users accessing The Haven and CRHTT services, with particular emphasis on:
- Utilizing clinical skills and judgment to assess needs and manage risks effectively
- Understanding clinical interventions, including education, care planning, and supportive discharge planning
- Working compassionately and creatively to identify therapeutic, evidence-based outcomes and care plans for those experiencing a mental health crisis
- Collaborating with carers, families, and partner agencies to ensure safety and well-being
You will work across urgent care services, responsible for discharge assessments and participating in multi-agency meetings.
