We have an exciting opportunity to join the Neighbourhood Mental Health Team (NMHT) as a Band 6 Pathfinder clinician. The role involves providing short-term clinical interventions for individuals accessing Pathfinder West Sussex services, particularly those whose mental health is deteriorating or those requiring support in their discharge from NHS mental health services.

Interventions include individual work, groups, workshops, and Recovery College. The focus of interventions will be on offering timely, proactive, preventative support and facilitating transitions between NHS and voluntary sector mental health services.

The post holder will work as part of an evolving team of clinically trained staff within Pathfinder West Sussex, collaborating with voluntary sector providers to offer clinical intervention and support to people with mental health difficulties. The role aims to build clear pathways between voluntary sector and NHS mental health services within the innovative partnership alliance, Pathfinder West Sussex, part of NMHT.

Experience of community and partnership working is required, along with the ability to work as part of a wider system, enabling solutions and working flexibly, adaptively, and creatively within a developing service in a specific locality.

As we work towards providing an integrated service across primary and secondary care, our community mental health services are being transformed to meet evolving access preferences. We collaborate closely with partners and 'experts by experience' to develop and improve our service offer.
