Overview

Be part of a pioneering new service transforming how we care for patients across Sussex. Remote Monitoring supports patients at home, helps prevent unnecessary hospital admissions, and delivers compassionate, timely care through innovative virtual solutions.

Role: Remote Monitoring Clinical Delivery Practitioner within Community Care. You will oversee utilisation of remote monitoring licenses and equipment across community services, support the development of future clinical pathways, and provide clinical advice and guidance to internal and external stakeholders including clinicians, care homes, acute trusts and primary care. You will support continuous evaluation and regular reporting of metrics. Although patient monitoring occurs remotely, this role is not fully remote. The base is negotiable but will be within an SCFT site, with travel across the SCFT footprint to provide clinical leadership and to embed the service.

Substantive post with review of ongoing role & responsibilities from year 2 as part of service review.

Main duties of the job

  • Being responsible for the expansion of remote monitoring licences across community care
  • Support and motivate staff engagement on remote monitoring
  • Develop new pathways, ensuring robust governance and planning
  • Ensuring service quality and performance targets are met
  • Promoting compassionate, patient-centred care

We are seeking a registered Nurse or Paramedic (NMC/HCPC) with:

  • Excellent and proven leadership skills

  • Knowledge of frailty, heart failure, COPD, and diabetes

Qualifications

Essential

  • Registered General Nurse or Paramedic – NMC or HCPC
  • Evidence of post registration experience
  • Educated to degree level

Desirable

  • Project management experience
  • Postgraduate qualification in health-related subject
