Health and Therapy Assistant – Home First, Chichester
Join the Sussex Community NHS Foundation Trust on a 7‑day, 365‑day Home First service providing step‑down intermediate care and acute reablement to patients discharged from hospital. You will support recovery, prevent re‑admission and help people achieve greater independence in their own homes.
What you will do
- Assist multi‑disciplinary colleagues with assessment and provision of care and reablement as prescribed in the personalised care plan.
- Take physical observations, perform venepuncture, administer medications, and carry out simple wound care and rehabilitation programmes.
- Observe changes in patients’ status, record information accurately and provide verbal updates to the team.
- Drive to patient homes using a full UK licence and access to a vehicle; the geographic area covered by the team requires reliable travel.
