The predicted salary is between 28800 - 48000 £ per year.
To process referrals in a timely manner, highlight and resolve any issues or incomplete information with proactive liaison with the referral single point of contact or referrers.
To ensure systems are in place to ensure appropriate therapy reviews are undertaken by team members.
To act as coordinator to ensure actions and tasks by members of the multidisciplinary have been completed in a timely manner.
To support allocation of appointments for the team.
To follow up and liaise with equipment providers.
To signpost and refer on to other community services.
Support data collection to facilitate audit and service development under supervision
The post holder will provide prompt, efficient co-ordination, support clinical co-ordinators with triaging and administration support to the service. They will also work within a multi-disciplinary and professional team to support implementation of short-term rehabilitation provision and track progress of interventions.
Liaise with equipment providers and follow up with equipment orders for the team.
