Surrey Heath Community Providers is seeking a Full-time Clinical Pharmacist to optimise medicines management across General Practice and Care Homes within the Surrey Heath Primary Care Network. Based across two practices, this role is five days per week.

The position is a training role requiring completion of a CPPE course. You will support practices to implement medicines management improvements, initiate and manage change, assist the PCN with medicines optimisation requirements, and undertake
