Overview


Full-time Clinical Pharmacist role in Medicine Management Optimisation across General Practice and Care Homes in Surrey Heath. Based across two practices within the Surrey Heath Primary Care Network. Five days per week.


Responsibilities



  • Support practices to implement effective medicines management and identify areas for improvement.

  • Initiate and manage change to optimise medicines use.

  • Support the Primary Care Network to implement medicines optimisation requirements of the Network Contract, Direct Enhanced Service for enhanced health in care homes, and other PCN medicines management responsibilities.

  • Undertake resident/patient-facing activities and medicines management support across registered populations.


Qualifications / Training



  • This is a training role requiring the successful applicant to complete a CPPE course.
