Surrey Heath Community Providers Limited is seeking a Clinical Pharmacist to join the expanding clinical pharmacy team.
You will work across one or more GP practices within the Surrey Heath PCN and support the PCN Care Homes Team to implement medicines optimisation as part of the Network Contract Direct Enhanced Service.
The role includes medicines management, risk stratification, and supporting care homes, with occasional evenings, weekends, and bank holidays as required.
