Surrey Downs Health & Care (SDH&C) focuses on an innovative care delivery model through distributed clinical leadership in Primary Care Networks, integrated community bedded services, and specialist services. The Clinical Lead/Community Services will provide clinical leadership to the PCN nursing and AHP workforce across the PCN and Federation.
The postholder will support the introduction of new ways of working, including:
- Providing day-to-day advice on complex patient care
- Liaising with other agencies as appropriate
- Supporting clinicians and giving advice on individual patient circumstances
- Establishing the framework for clinical and team PCN development
- Ensuring the delivery of excellent and safe care
- Supporting the development of new ways of working across PCN and between primary and community nursing services
The role includes supporting the Operational Manager and Lead GP in operational delivery and the development of new services integrated into the Primary Care Networks. This involves reviewing Datix incidents and developing action plans to prevent re-occurrence alongside policy review and development of clinical practice within the PCN.
The postholder will also hold ongoing clinical responsibilities, including undertaking home visits and managing complex patient care within the PCN or Federation, relevant to their professional background.
