Surrey Downs Health & Care is seeking a Band 7 Community Nursing Team Lead to join the ICP PCN team on a full-time (37.5 hours/week), 9 month secondment basis. The successful candidate will be welcomed into an established community nursing team that works closely with the wider multidisciplinary team across the PCN and Surrey Downs Health and Care. The organisation focuses on staff support and development, offering opportunities for further learning. Responsibilities

  • Lead and manage the District Nursing Team ensuring delivery of high-quality, person-centred care in the community.
  • Oversee assessment, planning, and implementation of care in line with agreed standards.
  • Maintain professional standards and contribute to clinical governance and risk management strategies.
  • Ensure staff development and participate in workforce planning, change management, KPIs, and audits.
  • Promote integrated care and collaboration across teams, ensuring respect, safeguarding, and high delivery of care to patients and staff.
  • Provide leadership within the District Nursing Team, assessing patient needs and developing, implementing, and evaluating a programme of care.
  • Facilitate educational programmes for basic, post-basic, and in-service training in liaison with the Learning and Development Team.
  • Lead and facilitate patient-focused, innovative research-based practice aligned with Surrey Downs Health and Care objectives.
  • Contribute to achieving Surrey Downs Health and Care's aims of delivering care closer to people's communities through Primary Care Networks, Community Hospitals, and other services.

The three GP federations: GP Health Partners, Dorking Health Care, and Surrey Medical Network (representing practices in the Surrey Downs area); Epsom and St Helier's University Hospitals NHS Trust; Surrey Council County. Historically there have been boundary lines between organisations providing care at home, in GP surgeries, and in hospitals, but the partnership aims to provide great care by working together. Surrey Downs Health and Care was formed to enable local people to receive the care they need in the right environment, bringing together expertise to improve patient care and access to support, care, and treatment. In bringing this partnership together, the organisation works to the same values to translate into better care for residents.
