Surrey Downs Health & Care is looking for Band 5 nurses to work at Epsom PCN. We are seeking nursing staff from all backgrounds, including newly qualified and experienced nurses, who are passionate about caring for people and making a difference in the community.
The role involves:
- Working as part of the team with lead responsibilities as negotiated with the District Nurse.
- Treating all patients as individuals, respecting their privacy and dignity.
- Assessing individual care needs and implementing holistic programmes of care.
- Involving, supporting, informing, and educating family/carers.
- Promoting health and wellbeing of the patient.
- Developing effective communication and liaison systems.
- Maintaining clear and comprehensive records according to SDHC procedures.
- Facilitating training and professional updating for team and students.
- Acting as an assessor and mentor to qualified and unqualified staff.
- Maintaining standards of evidence-based nursing care.
- Supporting and mentoring team members and participating in clinical supervision activities.
- Working without direct supervision while maintaining close liaison with senior team members.
- Being responsible for timely patient care and record keeping.
- Cooperating with other professionals and agencies involved in patient care.
- Being aware of and acting upon procedures to protect vulnerable individuals.
Qualifications:
- Registered Adult Nurse with NMC.
- Valid Driving Licence (to drive within UK).
Experience:
- Understanding of current issues affecting the NHS.
- Understanding of the Clinical Governance agenda.
- Community experience.
Other:
- Access to a vehicle for work purposes.
Skills:
- Ability to assess, deliver, and evaluate quality care.
- Safeguarding and DBS Checks.
