The Team works closely with carers, family members and partner organisations to enable residents to live their best possible life, ensuring that no one is left behind. We are a professional team made up of a variety of roles and backgrounds, and we support and encourage learning and development at both an individual and team level.

About the Role

We are looking for a compassionate and empathetic individual with a variety of skills and experiences to work with the people we support, enabling them to enjoy a home of their own and an environment in which they are supported to live a healthy and meaningful lifestyle.

Your role will be to assist qualified practitioners with a variety of tasks, including assessing people\’s needs and carrying out regular reviews. You will often be the first point of contact between the public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.

This post will be focused on the Integration part of our team focusing on supporting discharges from Kingston Hospital, and completing assessments in the community following a period with health funded support or reablement to identify any long term needs.

Proven ability to prioritise and manage multiple tasks effectively, ensuring timely and high-quality outcomes.
Proficiency in basic IT skills such as Microsoft Office, email and file management alongside a demonstrated willingness to learn and adapt to new software systems.

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. Reasonable adjustments where needed will be made for successful applicants who have a disability or long-term health condition to enable them to fulfil the requirements of the job.

An enhanced DBS \’Disclosure and Barring Service\’ check for regulated activity (formerly known as CRB) and the Children\’s and Adults\’ Barred List checks will be required for this role.
