About the Role
This is a great opportunity to step into a leadership role within a supportive, high-performing, and award-winning team. Based in Epsom Hospital, our team is known for its collaborative culture, commitment to professional development, and strong track record in delivering timely, person-centred hospital discharges. Our team is one of five Surrey County Council acute hospital teams within the Adult Social Care directorate. We support safe and timely acute hospital discharges, working closely with health partners, residents, and families to ensure people receive the right support at the right time. Patients are supported on discharge with packages of care to return to living in the community, or short-term placements, under the discharge to assessment framework. The role is to support discharge planning and post-discharge assessment within a set timeframe to maximise independence.
Responsibilities
- Manage and support the day-to-day running of a busy, dynamic team, prioritising daily challenges and expectations (e.g., rotas, allocation of work, duty function, and discharging planning meetings).
- Confidently make sound decisions that respond flexibly to changing needs in a high-pressure hospital team environment.
- Develop effective partnership relationships with individuals, carers and families, and foster positive relationships with professional health staff.
- Provide support to the Team Manager in delivering a high-quality social care service.
- Play a pivotal role in expediting discharges for vulnerable adults and provide specialist guidance and support to staff on managing complex case work.
- Supervision and support of staff development and performance to ensure continuous improvement within the team.
Qualifications and Experience
- Professional Social Work or Occupational Therapy degree and current professional registration with the relevant authority.
- Significant post-qualification experience, including some management experience in a social care setting, with a strong understanding of social care and health agendas.
- Applied knowledge of Adult Social Care legislation, including the Mental Capacity Act and the NHS Continuing Healthcare National Framework.
- Effective knowledge of safeguarding vulnerable adults procedures.
- Excellent communication skills to manage a team and develop effective relationships.
- Evidence of professional supervision provided to less experienced colleagues.
- Track record of inter-agency partnership working.
- Commitment to achieving positive, strengths-based outcomes that promote maximum community inclusion, personal choice and control.
- Willingness and ability to travel around the county to meet role demands.
Role Details
- Location: Epsom General Hospital (hybrid role; office base at least three days per week).
- Hours: Service operates 8am–6pm, Monday to Friday; 8am–4pm at weekends; role covers Monday to Friday.
- Salary: £54,636 per annum (36-hour week).
- Status: This is a secondment up until May 2026. Please discuss with your current Line Manager before applying.
