Overview

This role supports delivery of a 7 day specialist palliative care service at SASH. You will be required to work weekends to a rota. An experienced clinical nurse specialist is sought to join an established Macmillan Specialist Palliative Care team to support a 7-day visiting service at SASH.

The palliative care team provides advice and support to patients, carers and staff across the Trust on advanced disease management, pain and symptom control and end of life care. They liaise with community colleagues and local hospice, and provide education and support to staff caring for palliative care patients throughout the Trust. As a CNS within a multi-professional team, you will act as a key worker, liaising with primary care providers and relevant outside agencies to provide a coordinated and comprehensive service.

Responsibilities

  • Deliver a 7-day visiting palliative care service as part of a Macmillan Specialist Palliative Care team.
  • Act as a key worker within a multi-professional team, coordinating care with primary care providers and external agencies.
  • Provide education and support to staff caring for palliative care patients across the Trust.
  • Utilise clinical knowledge and experience to educate as appropriate and act as a resource for all staff and other healthcare professionals.
  • Contribute to a well-established and expanding Cancer Nursing Service, with opportunities for clinical supervision and professional advancement.

Requirements / Qualifications

  • Experience as aClinical Nurse Specialist (CNS) or equivalent in palliative care.
  • Confident, self-motivated and with excellent communication and influencing skills.
  • Ability to work as part of a team and provide leadership in education and clinical practice.

Working Arrangements and Environment

Surrey and Sussex Healthcare NHS Trust operates a 24-hour service, 365 days a year. All nursing, clinical and operational roles may involve 24/7 shift work unless otherwise specified. Appointments are made to the first pay step within the band at start (subject to previous NHS continuous service). Pro rata salary applies for part-time roles in line with AfC terms and conditions.
