Overview
An exciting opportunity for a RNLD / RMN to join the Intensive Support Service within the Trust\’s Learning Disabilities Directorate. The Intensive Support Service is an innovative team created to support people with learning disabilities and complex needs, in the community and inpatient settings. The Multi-Disciplinary Team includes Nursing, Senior Support Workers, Occupational Therapy, Speech & Language Therapy, Psychology, Psychiatry, Therapy Assistants and Assistant Psychologist. The service is designed to support people whose mental health and/or behaviour support needs put them at risk of admission to hospital or placement breakdown. We work closely with the Community Team for People with Learning Disabilities in the East and West. The post offers opportunities for development and joint working with other disciplines and agencies in the community crisis team and the inpatient unit.
Responsibilities
- Demonstrate expertise and specialist nursing skills in learning disability settings, including crisis intervention, alternative communication, assessment and management of people with additional mental health needs, epilepsy, medication management, challenging behaviour and related conditions.
- Actively involve people in developing and agreeing their own care plans, advocate for them, and ensure dignity, choice, self-esteem, and consideration of racial, cultural, sexual and religious needs and preferences.
- Carry out comprehensive assessments and plan, implement and evaluate nursing interventions as part of a holistic approach across different settings.
- Practice within the Positive Behaviour Support framework.
- Undertake comprehensive risk assessments and develop effective risk management plans.
- Work in partnership with acute and primary care services and other healthcare providers.
- Establish and maintain effective relationships with colleagues, other agencies, people who use services and their families.
Qualifications and Skills
- Registered Nurse with learning disabilities (RNLD) or mental health (RMN) qualification.
- Experience working with people with learning disabilities and complex needs in community and inpatient settings.
- Skills in crisis intervention, communication, medication management, and handling challenging behaviour; ability to work across disciplines and agencies.
- Commitment to person-centered care, advocacy, and safeguarding of dignity and diversity.
