PCN Care Navigator (0.4 FTE) – Patient Care Coordination

Surbiton Health Centre PCN is seeking a Care Coordinator to join the Primary Care Network as a part-time role (0.4 FTE) and share duties with an existing staff member, ideally on Thursdays and Fridays with some flexibility.

You will work within the PCN MDT, coordinating patient care with GPs and other primary care staff, signposting services, and ensuring timely, patient-centred support across health and care pathways.
