About the Role

We are seeking a passionate, highly skilled Domiciliary Care Nurse to take a pivotal role in overseeing complex TDDI packages of care within the community.

What You’ll Be Doing

  • Provide clinical oversight and governance for TDDI and complex care packages delivered in service users’ own homes
  • Carry out initial and ongoing clinical assessments, reviews, and risk evaluations
  • Train, delegate, and assess competency of care staff in a range of clinical procedures, ensuring compliance with NMC standards and organisational policy
  • Develop, review, and implement individualised clinical care plans and protocols
  • Act as a clinical escalation point for carers and managers, offering guidance, reassurance, and problem‑solving support
  • Liaise with families, commissioners, GPs, community teams, and allied professionals to ensure joined‑up care
  • Support audits, spot checks, and quality assurance activity linked to clinical care delivery
  • Contribute to service development, best practice, and continuous improvement

Clinical Procedures You Will Oversee & Train In

You will provide training, delegation, supervision, and review for carers undertaking procedures including (but not limited to):

  • PEG feeding and enteral nutrition
  • Tracheostomy care and suctioning
  • Catheter care (urethral and suprapubic)
  • Bowel management and evacuations
  • Medication administration (including complex regimes)
  • Pressure area care and wound management
  • Infection prevention and control
  • Emergency protocols and deterioration management

What We’re Looking For

Essential:

  • Registered Nurse (RGN/RNLD/RSCN) with a current NMC PIN
  • Proven experience in community, complex care, or clinical training roles
  • Strong understanding of TDDI frameworks, delegation, and accountability
  • Confident trainer with the ability to upskill and inspire care staff
  • Excellent communication and leadership skills
  • Ability to work autonomously across community settings
  • Full UK driving licence and access to a vehicle

Desirable:

  • Experience in domiciliary or complex care provision
  • Teaching, mentoring, or assessor qualification
  • Knowledge of CQC‑regulated environments
